I would like you to send me the following documents:

Copy of Birth Certificate of you, your spouse (if any) and your children (if any). (If you do not have the appropriate birth certificate, please find enclosed draft of “Affidavit of Birth”. We need two Affidavit of Birth for each individual who does not have birth certificate. ) Also, please provide non-availability birth record from the Municipality Corporation.

Note: Affidavits should be from affiants at least 10 years older than the even he/she is attesting to. The affiants may be relatives, but AFFIDAVITS MADE BY PARENTS ARE OF LITTLE PROBATIVE VALUE BY THEMSELVES IN ESTABLISHING SIBLING RELATIONSHIP AS THEY ARE SELF SERVING. The affiant need not to be US Citizens, but should contain the following information regarding the event; I. His/Her full name and address; II. Date and place of Birth III. Relationship to you IV. Full information concerning the event V. Complete details concerning how he/she acquired knowledge of the event.

Copy of I-94s, I-797’s and all INS related documents.
Six green card type photographs. (Please make sure that you get these photographs from an INS authorized photographer). Please search the websites and/or yellow pages for your local photographers.
Bank letter (draft enclosed).
We need a medical check up performed by an authorized Physician. To obtain by telephone the names and phone numbers of the designated civil surgeons in your area, please call USCIS' National Customer Service Center at 1-800-375-5283 (please note that these medical reports need to be in separate envelope for each family member).
